How do I place my membership on hold or cancel it?

** FIT4MOM NWI MEMBERSHIP cancellation requires 30 days notification of change in active status. All requests need to be received in writing 30 days before the next payment is due AND must be confirmed by Email from janewallace@fit4mom.com. Membership Status Change paperwork can be picked up in person from a FIT4MOM Northwest Indiana location or Our Village event. Failure to give 30 days notice may result in a $50 cancellation charge.
